Gastric adenocarcinoma of fundic gland type (chief cell predominant type): proposal for a new entity of gastric adenocarcinoma (Am J Surg Pathol 2010;34:609-619)
- これまで主細胞への分化(pepsinogen-I陽性)を示す胃腺癌の報告は１例のみであったが，最近，UeyamaとYaoらが10症例をまとめてAm J Surg Pathol 34:609-619, 2010に発表した．臨床病理学的な特徴は，胃の上部(U領域)の萎縮や腸上皮化生に乏しい胃底腺粘膜に発生し，小さいながらもSMへ浸潤するが，生物学的悪性度が低く(脈管浸襲陰性，低いKi-67標識率，p53陰性)，予後良好という点である．胃癌における既存の概念のいずれとも異なるため，gastric adenocarcinoma of fundic gland type (GA-FG) (chief cell predominant type)という名称で新しい胃癌の組織型として提唱している．
- 彼らの報告ではごく早期の病変のみであり，その発育進展様式の解明や予後などの臨床事項のさらなる検討および胃底腺型胃癌の亜型であるparietal cell carcinomaやmixed-chief and parietal cell typeの症例の集積や解析が今後の課題としている．
